Right to Notice

As a patient, you have the right to adequate notice of the uses and disclosures of your protected health information. Under the Health Insurance Portability and Accessibility Act (HIPAA), Lisa Porter O.D. can use your protected health information for treatment, payment and health care operations.

Your Authorization

Most uses and disclosures that do not fall under treatment, payment, health care operations will require your written authorization. Upon signing, you may revoke your authorization (in writing) through our practice at any time.

Emergency Situations

In the event of your incapacity or an emergency situation, we will disclose health information to a family member, or another person responsible for your care, using our professional judgment. We will only disclose health information that is directly relevant to the person's involvement in your healthcare.

Marketing

We will not give, sell, or trade your health or personal information to any other party except those listed below that are protected by law. We may use your personal information for internal marketing purposes (i.e. practice newsletters, promotional events like Open Houses or trunk shows, reminders for appointments, or special occasions such as holiday greetings) without your written permission but will not release any health information under such circumstances.

Dr. Lisa L. Porter grew up on a dairy farm in South East Idaho, dreaming one day of becoming an Eye Doctor. Now with her Doctorate Degree and several years' experience earning several awards, Dr. Porter still loves doing what she does best - helping patients with all their eye care needs. Compassionate and qualified, she loves working with people of all ages including infants and toddlers and is looking forward to serving you.
